Code of Conduct – Parents
It is the intention of this pledge to promote proper behaviour and respect for all participants within the Ottawa Ice Lightning Hockey Club (OILHC). All parents/players must sign this pledge before being allowed to participate in the events organized by OILHC. Failure to observe the Code of Conduct may result in OILHC disciplinary action.
1. I will not force my child to participate in hockey.
2. I will remember that my child plays hockey for his or her enjoyment, not mine.
3. I will encourage my child to play by the rules and to resolve conflict without resorting to hostility or violence.
4. I will teach my child that doing one’s best is as important as winning so that my child will never feel defeated by the outcome of the game.
5. I will make my child feel like a winner every time by offering praise for competing fairly and hard.
6. I will never ridicule or yell at my child for making a mistake or losing a game.
7. I will remember that children learn by example. I will applaud good plays and performances by both my child’s team and their opponents.
8. I will never question the official’s judgment or honesty in public. I recognize officials are being developed in the same manner as players.
9. I will support all efforts to remove verbal and physical abuse from children’s hockey games.
10. I will respect and show appreciation for the volunteers who give their time for my child.
Code of Conduct – Players
1. I will play hockey because I want to, not because others or coaches want me to.
2. I will play by the rules of hockey and in the spirit of the Game.
3. I will control my temper – fighting or “mouthing-off” can spoil the activity of everyone.
4. I will respect my opponents and teammates, and to be a true team player.
5. I will remember that winning isn’t everything – that having fun, improving skills, making friends and doing my best is also important.
6. I will acknowledge all good plays and performances – those of my team and my opponents.
7. I will remember that coaches and officials are there to help me. I will accept their decisions and show them respect.
Code of Conduct – Parents and Players
▪ We (parent and player) agree to abide by the rules, regulations and decisions as set by OILHC.
▪ We (parent and player) agree to abide by the principles of this CODE as set by OILHC.
Program Agreement and Release from Liability – Ottawa Ice Lightning Hockey Club
As parent or guardian of the child who is enrolled to this Program, I understand and accept that both myself and the child will be required to adhere to a code of sportsmanlike conduct that respects not only his/her own personal safety, but also the personal safety of his/her fellow players from all teams, coaches, bench staff, referees, and arena officials. In respecting the personal safety of others, both I as the player’s parent, and the child agree to abide by the following rules, and accept that any violation of same could result in both my and / or the child’s removal from play and from the arena either temporarily, or permanently for the duration of the intended season. As this is not regular season play, I agree and accept that the decision of the coach in excluding myself or the child will be fair, reasonable, and final, and will not be appealable to any third party. I further accept that this may further involve the forfeiture of any fees, and without any refund that I have paid for the enrolment of my child, as said fees are necessary for the support of the entire program. While applying logic and reasonable behaviour in my and my child`s conduct generally, both myself, and further the child, together agree as follows:
1. To use respectful language in his/ her, my dealings with players, officials, staff and other parents at all times;
2. To refrain from behaviours that would be considered as bullying, harassing or annoying;
3. To refrain from violent play at all times; and
4. To bring honour and respect to myself and my child at all times while involved in the OILHC Program.
As I fully understand that the parties who have organized the program are volunteers (whether a stipend is paid for their time or not), I recognize that while diligence and common sense will be applied by the organizers, coaches and other volunteers at all times, that accidents can and do happen, and are generally outside of the control of these persons. I accordingly waive all right of action on my behalf, and any party who could advance a claim under section 61 of the Family Law Act, and further on behalf of the minor child, as a condition to, and consideration for, their enrolment within the hockey program. Any coach, volunteer or organizer involved with the OILHC Program may rely on this Waiver as a Full and Final Release and Discharge from liability, unless the conduct of said person or persons would be deemed by a Court of competent jurisdiction in Ontario as gross negligence, or criminal in nature as provided for under the Criminal Code of Canada. I acknowledge that my and the child’s acceptance below have not been made until after this agreement has been fully read and understood by me. I also acknowledge that I was permitted an opportunity to delay in signing this document until I was able to obtain legal advice, and that my and my child’s acceptance have either been applied to this document because I obtained such advice, or I chose to waive said advice.
Multimedia Release Form – Ottawa Ice Lightning Hockey Club
As the parent(s) or legal guardian of the participant(s) of the Ottawa Ice Lightning Hockey Club (OILHC), I understand that this form states that I voluntarily and without compensation irrevocably grant to the OILHC the right to use of our and the child(ren)’s likeness (“Likeness”) for the purpose of promotion, publicity, advertising, or other manner or media by OILHC and its organizers, authorized representatives, volunteers, coaches, and other staff. Likeness shall include, but not be limited to, photographs, sound and/or video recordings, films, broadcasts, brochures, publications, reports, web pages, promotional materials or any other audio-visual, electronic, printed, tangible work in any media or format, now known or hereafter to become known, and/or reproductions of any of these. I further realize that all likeness may be used by OILHC sponsors for marketing purposes. I hereby release, discharge and agree to save harmless the OILHC from claims relating to rights of privacy,

COVID-19 / Influenza virus (Type A/B) Declaration Form – Ottawa Ice Lightning Hockey Club
As the parent(s) or legal guardian of the participant(s) of the Ottawa Ice Lightning Hockey Club (OILHC), I hereby declare that I fully understand although more public health measures are lifted, COVID / Influenza Type A/B is still present and by participating OILHC activities, there exist the risk of transmission and the risk of infection and severe outcomes from COVID / Influenza Type A/B . I hereby declare that I discharge and agree to save harmless the OILHC from claims relating to any outcomes from COVID / Influenza Type A/B that may relate to OILHC activities.
I have had sufficient time to review and seek explanation of the foregoing provisions.
I have carefully read them, understand them fully, and agree to this declaration.
